A shared concern for youth is not one combined service.

Reach One Clinical Services logo
  • CGT sports, after-school activities, and mentorship are not automatically therapy.
  • Submitting CGT interest does not create a Reach One clinical record.
  • Clinical services require separate assessment, consent, eligibility, qualified staff, and clinical responsibility.
  • Families retain provider choice.
  • No routine clinical information sharing through public CGT forms.
  • Reach One’s accreditation or professional authority does not transfer to CGT.

CONNECTING TO SUPPORT

Complete the handoff without exposing the story.

With the appropriate permission, an assigned staff member may help a caregiver understand the next step, contact a chosen provider, confirm scheduling, and track whether the connection occurred. Community records should not copy clinical notes or unnecessary sensitive details.
